Dear Dr. Host’s patients: Thank you for continuing your care. I am very excited to invite you to enjoy the experience of personalized, high-quality primary care. I am Board Certified in Family Medicine, Geriatrics, and Lifestyle Medicine. You will receive high-quality services and additional services that you received with Dr. Host at the same price with the same people and in the same office to continue primary care. You will be happy to know that I and Dr.Host, covered for each other for several years during emergencies and vacations.

We are happy to inform you that you can set up a FREE 30-MINUTE visit with Dr. Gokula. This visit focuses on getting to know each other and learning about your health. After the visit, we will get wellness labs to check for inflammation and insulin resistance in your body by doing lab work with Quest labs, included in your membership fee. We will create a personalized plan to help reduce inflammation and insulin resistance in a holistic approach addressing body, mind, and spirit to bring physical, mental, and social well-being. 

THE ANNUAL FEE IS THE SAME AS WITH DR.HOST. All other prices are the same (ADDITIONAL CARE SERVICES, A LA CARTE (for member patients), MEDICATIONS AND TESTING, MEDICATIONS DISPENSED IN OFFICE. You will continue to receive the same benefits with Dr.Host for your monthly membership.
